Dauphin Swim Team

As the parent or guardian, I give my permission for my child to participate in the Dauphin Swim Team Program. By participating in the Dauphin Swim Team, information about our swimmers will be exchanged between teams, some of which will end up public domain. The following information, and only the following information shall be acceptable for publication in any form, including but not limited to the World Wide Web or publicly posting at meet venues, regarding any registered swimmer without express written authorization from the swimmer's parent or guardian: -Name -Age -Team Affiliation - Time and Place in Event / Heat - Any Awards or honors Since most of the leagues meets are held in public areas it is understood that meet participants may be photographed or videotaped during the events. These images may appear in publications, websites or other media sources. Dauphin Swim team, and all teams within the league have agreed to a policy which will ensure that while pictures of minors may appear on websites, no names will accompany such pictures. Use of audio or visual recording devices including a cell phone is not permitted in the changing areas, rest rooms or locker rooms. Physical Health - I am fully aware that the program he/she us about to participate in, involves physical endurance. Further I attest that he/she is in good physical health and he/she has no physical or health restrictions which would limit him/her from participating in the program. Liability - The undersigned does hereby and for my heirs, executors, administrators, successors and assigns, release and forever discharge the Dauphin Swim Team, sponsoring organizations, their directors, administrators, managers, coaches, officials, staff, agents and members from any and all claims, actions, expenses, and compensations whatsoever that may arise or accrue out of the above mentioned activity. The undersigned hereby declares that he/she voluntarily assumes all risks inherent and further declares that he/she will hold the directors, administrators, managers, coaches, officials, staff, agents and members of the Dauphin Swim Team, and sponsoring organizations harmless for any and all injuries, whatever the extent or nature, that may arise from this activity, and during transportation to and from the activity. The undersigned hereby declares that no promise, inducement or agreement not herein expressed, has been made to the undersigned, and that this release contains the entire agreement between the parties, and that the terms of this release are contractual and not a mere recital. The undersigned hereby declares that he/she is signing this release of his/her free will, without threat of coercion. I have read and fully understand and concur with the above policy and waiver of liability. 

Swimmer Responsibilities The coaches are in charge of swim practices and meets. Swimmers will give the coaches their respect and best effort while participating as a member of the swim team. Swimmers at practice are expected to be in the pool area with the coaches. Neither disrespectful and profane language nor dangerous and destructive behavior will be tolerated. Coaches have the authority to remove any swimmer who engages in these behaviors. The Head Coach has the authority to define these behaviors. Repeated incidents will result in expulsion from the team according to the disciplinary schedule below. Disciplinary Schedule First offense A warning will be given to the individual. Second offense A second warning will be given and the parents of the offender will be notified and asked for their assistance in correcting the behavior. Third offense Constructive discipline will be applied at the discretion of the coaches, for example, swimming extra laps, etc. Fourth offense Suspension or expulsion from the team at the Head Coach’s discretion. Swimmers will abide by the health and safety rules of the Dauphin Recreation Authority, who owns the pool and surrounding property. These rules are posted on the property and are considered part of this Policy by reference. Parent Responsibilities Parents at practices will help supervise and assist with discipline of swimmers and nonswimmers. Parents are responsible to see that their children arrive at practices and meets on time and are picked up promptly after those practices and meets All swimmers and nonswimmers within the pool enclosure are subject to the authority of the team coaches and the Dauphin Swimming and Diving Association Board members. Parents acknowledge that any child present in the pool enclosure is subject to this Behavior Policy and agree to support the coaches and Board members in enforcement of this Policy. Parents will abide by the health and safety rules of the Dauphin Recreation Authority, who owns the pool and surrounding property. These rules are posted on the property and are considered part of this Policy by reference Nonswimmers who consistently disregard the Behavior Policy will be banned from the pool enclosure during team practices. I support the Behavior Policy of the Dauphin Swimming and Diving Association. I will abide by this policy in order that everyone involved with the team will have a safe and enjoyable experience this summer. Non-Member families will be charged a $25 non-member fee at checkout

There will be a variety of fundraising options which will offer credit amounts toward a $50/Family Fundraiser Fee.  If you plan to fundraise - do not elect this fee.  Any balance on accounts will be billed at the end of the season.  If, however, you do not plan to fundraise at ALL, please select this fee and it will be added at checkout
